O personagem Dr. John foi baseado em um praticante de vodu do século 19, Dr. John Monatee, do Senegal, e foi inventado para Ronnie Barron, parceiro de Rebennack. Após Ronnie deixar a banda na qual tocavam, Malcolm Rebennack assumiu o personagem que moldaria sua vida e carreira.
Sua carreira musical profissional começou em Nova Orleans na década de 1950. Ele inicialmente tocava guitarra em bandas locais. Sua carreira como guitarrista chegou ao fim quando o seu dedo anelar esquerdo foi ferido por um tiro quando estava defendendo o cantor e tecladista Ronnie Barron, seu amigo de escola. Após a lesão foi aprender a tocar piano com o Professor Longhair , foi uma influência importante sobre o seu estilo.
Rebennack ganhou fama como um artista solo, que tem início no final dos anos 1960, com música, que combina o estilo de Nova Orlens com ritmo e blues, rock psicodélico.
